Job description

POSITION SUMMARY:

The Renal Dietician is responsible for the provision of dietetic services to facility clients / patients in accordance to an established physician plan of care, and under the supervision of a Nephrologist and registered nurse (as dictated accepted standards of care, policy and state law). This delegation of responsibility is reviewed on a routine basis by the patient’s Nephrologist. All facility staff, including the Dietician, must demonstrate competency required to serve the dialysis population. The Dietician will demonstrate competency in dialysis care by producing a score of 80% or greater on the competency exam provided by Quality Dialysis One, LP. He or she reports to the Administrator.

Qualifications

  • Candidate must possess a high school diploma (or equivalent).
  • Candidate must provide evidence of the successful completion of a dietetic and clinical nutrition program.
  • Candidate must be registered with the American Dietetic Association and the Commission on Dietetic Registration.
  • Candidate must possess a minimum of one-year of professional work experience in clinical nutrition as a registered dietician after successful completion of the registration exam administered by the Commission on Dietetic Registration (experience with renal patients preferred).
  • Candidate must have a valid State of Texas driver's license with available and dependable transportation (and proof of current automobile liability insurance).
  • Candidate must possess the ability to meet minimum facility hiring standards including evidence of current cardiopulmonary resuscitation (CPR) certificate (and a willingness to perform such procedures in the home setting).

Duties and Responsibilities

  • Assess the patient’s nutritional needs with the appropriate measurement for mineral and bone disease protocols; Also included are weight changes, blood chemistries, dialysis adequacy, and medication regimen (which affect nutritional status and may cause potentially adverse nutrient interactions).
  • Manage 100 patients (with a maximum of 125 patients) – as per TDH guidelines.
  • Participate in the patient’s team review (PCC) and QAPI activities.
  • Recommend therapeutic diets in consideration of cultural preferences and changes (based on the patient’s nutritional needs after consultation with the nephrologist).
  • Referring patients for assistance with nutrition resources (such as financial assistance, community resources, and in-home assistance).
  • Provide ongoing counseling services to patients and family members concerning their dietetic requirements
  • Provide education material regarding mineral and bone disease management.
  • Maintains and submits required documentation relative to services delivered to the patient (within the guidelines of facility policy).
  • The collection of objective and subjective data to assess nutritional status will occur within 2 weeks (or 7 treatments) from admission to the facility, whichever occurs later. A comprehensive assessment (with an educational component) will be completed within 30 days, or 13 treatments, from admission to the facility, whichever comes first.
  • A nutritional reassessment will be completed initially, every 90 days, and annually (may also be done more often, if indicated).
  • Provide nutritional services at the facility during scheduled treatment times.
  • All verbal or telephone orders relating to bone and mineral metabolism shall be received by a renal dietitian, licensed nurse or physician assistant. Orders relating to dietetic service(s) may be received by a licensed professional (renal dietitian / registered nurse) and countersigned by a facility physician unless otherwise specified.
  • Demonstrates accountability in completion of assigned duties (in patient care and other related office activities).
